Plain-English Guide

What Is an Evidence of Coverage (EOC)?

An evidence of coverage, or EOC, is the carrier document that describes what a health plan covers, how benefits work and what is excluded. It is often confused with the summary of benefits and the summary plan description. This guide explains how they differ.

Three Documents

EOC, SBC and SPD side by side

EOC: the full terms

The detailed carrier document, sometimes called a certificate of coverage, with benefits, exclusions, appeals and definitions.

SBC: the short summary

A short, uniform summary plans and insurers must provide, built to let people compare coverage.

SPD: the ERISA plan description

The summary plan description is a separate document required for ERISA plans.

Using Them

What to do with each document

Check what is covered

Use the EOC to confirm how deductibles, copays and coinsurance apply to a service.

Do not mix up the SPD and the benefits guide

A benefits guide and a carrier EOC are not the same as an SPD.

Help employees find answers

At open enrollment, point employees to the SBC for comparison and the EOC for the details.

Plain-English definitions for the terms employers and employees ask about.

Going Deeper

The EOC holds the final word on coverage.

When a plan summary and the full document seem to disagree, the full document controls. That is why employers keep the current EOC on file and give employees access to it.

Whether a carrier document also meets the requirements for your ERISA summary plan description is a separate question. We review that with you, so there is no gap in your plan documents.

Common Questions

Questions we get

What is an evidence of coverage?

The carrier document that describes the benefits, exclusions and rules of a health plan.

Is an EOC the same as a certificate of coverage?

They are often used for the same thing. Insured group plans may call the document either one.

What is the difference between an EOC and an SBC?

The SBC is a short, standardized summary used to compare plans. The EOC is the full document with the detailed terms.

Is an EOC the same as a summary plan description?

Not necessarily. The SPD is an ERISA document with its own requirements. Ask your broker whether your carrier document meets them.
