Where it does apply, the clocks are fixed: thirty days from the employer to the plan administrator, fourteen more from the administrator to the individual, sixty days for that person to elect and forty-five further days to pay the first premium, with a thirty-day grace period on later payments.

Nothing in that sequence is discretionary, and the first step is the one employers miss, because it depends on somebody noticing that a qualifying event occurred and telling the administrator promptly.
