Cigna Pathwell Bone and Joint steers spine, hip, knee and shoulder care toward designated providers with a rich surgery benefit.
Pathwell Bone and Joint is a condition-specific care program, not a wellness app. It combines navigation with a benefit design that rewards members for using designated providers.
What the program is and what members do.
What the program is
Cigna describes Pathwell Bone and Joint as a condition-specific care program that helps employees with spine, hip, knee or shoulder pain get on the right treatment path, avoid unnecessary surgeries and reduce the organization’s spend. Cigna calls musculoskeletal conditions one of the top medical spend categories for employers.
What members do
Each member works one to one with a Clinical Care Advocate by digital messaging or phone, and uses a guided digital experience with activities, assessments and treatment guidance. Cigna says it uses predictive models to identify members at risk for surgery up to a year in advance and reach out. Advocates are nurses and physical therapists who do not give medical advice.
The surgery benefit
If surgery is needed, Cigna says a Benefit Specialist helps members qualify for a zero or low-cost surgery benefit and a travel benefit. Members use designated in-network orthopedic surgeons and neurosurgeons that meet Cigna criteria. Cigna’s footnotes say the plan pays 100 percent of eligible expenses for non-HSA plans, and after the deductible for HSA plans.
Eligibility, funding and plan design.
Who can use it
Cigna sends employers to their representative to begin. Product availability varies by location and plan type. The program is described for employer plans with Cigna Health and Life Insurance Company, and the page does not state a group-size cutoff, so eligibility needs to be confirmed.
How it is funded or sold
This is a benefit design plus a navigation service, so the employer’s plan pays the richer surgery benefit when members meet program participation requirements. Cigna does not publish a fee on its employer page. We would ask how the program is priced and whether it changes the plan’s cost sharing.
Where it fits in a plan design
It fits groups where musculoskeletal claims are a visible share of spend. The benefit design is the lever: members are rewarded for choosing designated providers, which changes how they behave at the point of care. Employers should weigh it against any stand-alone MSK vendor they already use.
What to settle before relying on it.
Which Michigan surgeons and facilities are designated?
The benefit only works if members can reach designated providers. Cigna says travel benefits apply for surgery with a designated provider more than 60 miles from home. We would ask for the Michigan designated list and how many are within reasonable distance of the employer’s workforce.
What are the participation requirements?
Cigna says the surgery benefit applies when program participation requirements are met. We would ask exactly what members must do, what happens if they skip the steps, and how that is communicated to members before they need surgery. A benefit with unclear conditions generates complaints and disputes.
How does this interact with our deductible and any HSA?
Cigna notes that HSA plans pay 100 percent after the deductible. We would model what the member pays under the group’s plan, whether it overlaps with an existing MSK vendor, and whether the employer expects savings. Cigna’s figures on MSK spend are its own analysis.
Common questions about this program.
Is Pathwell Bone and Joint a wellness app?
No. Cigna calls it a condition-specific care program with clinical navigation, designated providers and a surgery benefit design.
Does it cover surgery at no cost to the member?
Cigna describes a zero or low-cost surgery benefit when program participation requirements are met, and says HSA plans pay after the deductible. Details depend on the plan.
Who are the care advocates?
Cigna says they are nurses and physical therapists with a current license in at least one state who do not provide medical advice in that role.
