Pharmacy Benefit Consulting

Specialty drugs are a few claims that behave like underwriting.

A small number of members on high-cost therapies can move a renewal more than any plan design change. We find those claims as they develop, manage where and how they are filled, and put them in front of the stop-loss carrier and the renewal before either one surprises you. Specialty spend is tracked every quarter in our health and cost strategy.

Where the Cost Comes From

The drug is only one of four prices you pay.

The same therapy can cost very different amounts depending on decisions nobody at the employer is asked to make.

The Benefit It Runs Through

Many specialty drugs, especially infused ones, are billed under the medical plan rather than the pharmacy benefit. Pharmacy discounts and rebates do not apply there, and the claim often escapes the pharmacy review entirely. Medical-benefit specialty drugs need their own specialty drug management review.

The Site of Care

An infusion given in a hospital outpatient department can cost far more than the same drug given at a free-standing infusion center or at home. The clinical result is the same; the invoice is not. Site-of-care programs are one of the most direct specialty drug savings levers.

The Product

Where a biosimilar or a lower-cost equivalent exists, the formulary decides whether it is preferred. A rebate on the original product can make the more expensive choice look cheaper on paper. Net cost, not list price or rebate, is the fair way to compare a biosimilar with the original product.

The Channel

Specialty pharmacies owned by the pharmacy manager are often required by the contract. Whether that pricing is competitive is a question the contract should let you test. Specialty pharmacy pricing and channel requirements are terms worth testing in every PBM contract review.

The Tools, Honestly Assessed

Every specialty program saves the plan money by changing something for a member.

We weigh each one on both sides of that ledger before recommending it.

Clinical Review and Site of Care

Prior authorization and site-of-care steering are the most reliable savings. They also add a step before treatment, so the process and the exceptions need to work for the member, not just the budget. A specialty drug program that frustrates members costs goodwill that does not show on the invoice.

Manufacturer Copay Assistance

Programs that capture manufacturer assistance can lower plan cost sharply. How that assistance counts toward the member’s deductible and out-of-pocket maximum decides whether the employee pays more later in the year. We make that trade explicit. This is often called copay accumulator or maximizer design.

Carve-Out Vendors

Some vendors remove specialty drugs from the plan and source them another way. The savings can be real; so is the risk to the member if the sourcing fails. We read how the program works when it does not. The fallback plan matters.

Through the Year

A therapy that starts in month four is a renewal event in month ten.

We track high-cost claims monthly so each one is a known quantity by the time it matters.

Monthly Watch List

New specialty starts and high-cost claims flagged from pharmacy and medical data together, so an infused drug billed under medical is not missed. Watching medical and pharmacy specialty claims together is what lets a new high-cost therapy be managed before it reaches renewal.

Stop-Loss and Disclosure

Known high-cost therapies are reported where the stop-loss contract requires it and disclosed accurately at renewal, so a reimbursement is never put at risk. Accurate specialty drug disclosure to the stop-loss carrier protects the plan’s reimbursements when a high-cost claim arrives.

The Renewal Conversation

By renewal the large claims are already modeled: which ones continue, which end, and what the carrier’s trend assumption should and should not include. Modeling specialty drug claims ahead of renewal is how a carrier’s trend assumption gets challenged with evidence.

Common Questions

What employers ask about specialty drugs.

Should we cover GLP-1 drugs for weight loss?

It is a budget decision and an employee-relations decision at once. We model the cost on your own population and the options in between, such as coverage with clinical criteria, before you decide. GLP-1 coverage decisions can be revisited each plan year as prices and evidence change.

Can we exclude a drug to control cost?

Some exclusions are allowed and some create compliance problems or real hardship. Any exclusion should come with an exceptions path and a plan for current users. Before excluding a specialty drug, check the ACA, mental health parity and state rules that may apply to your plan.

Does a fully insured group need any of this?

Less of the contract work, but the same visibility. Specialty claims drive a fully insured renewal too; knowing them is how a renewal gets challenged. Specialty drug data is some of the most useful evidence a fully insured employer can bring to a renewal negotiation.
