One Claimant Can Decide Your Year
It is tempting to think of a health plan as an average, and for a large employer it broadly behaves like one. Below a few hundred enrolled subscribers it does not. A single serious diagnosis, a premature birth, a course of specialty medication — any of these can dominate a small group’s claims for a year, and the arithmetic is not gradual.
What decides whether that lands on you is the pooling attachment point: the level above which a claim stops being charged to your experience. Below the point, it is yours. Those points moved up for the current year, which means more of each large claim now stays inside your own numbers than was the case before.
It Works in Both Directions, and That Is the Useful Part
A high-cost claimant leaving the experience period can swing a renewal down as sharply as their arrival swung it up. We have seen a group move by close to thirty points between consecutive years on exactly that basis, with no negotiation involved at all.
Which is why a decrease deserves an explanation rather than a celebration. If your renewal fell and nobody can tell you why, the honest possibility is that it fell for a reason that will reverse, and planning around it as though it were permanent is how the following year becomes a shock.
