Carrier Wellness Programs

Every major carrier sells wellness. What reaches your employees depends on size and funding.

Each carrier below publishes an employer wellness offering. What your group can use depends on its size, whether the plan is fully insured or self-funded, and which tier the carrier will sell you. This page describes each program as the carrier describes it. Looking for condition programs? See Carrier disease management programs → Want fitness and member discounts side by side? Ask What Your Carrier Offers

How To Read This Page

Platform, incentive and funding are three different questions.

Platform Versus Incentive

A platform is the app, health assessment and coaching. An incentive is the money or plan-design reward for using it. Many carriers include a platform and sell incentives separately.

Funding Changes the Answer

Some programs are described as open to fully insured and self-funded groups. Others are described for large groups or self-funded plans. Always confirm what your funding type can buy.

Sources and Dates

Every description here comes from the carrier’s own employer pages as of October 2026. Programs change, so confirm current details with the carrier before relying on them.

Blue Cross Blue Shield of Michigan and Blue Care Network

A base program in every plan, with rewards for self-funded groups.

Blue Cross says its Well-Being Base program is built into every BCBSM and Blue Care Network medical plan, fully insured and self-funded alike. Self-funded customers can add reward programs on top. Blue Cross’s Medicaid plan is covered in the last card. For care management and condition programs, see our page on Carrier disease management programs →

Blue Cross Well-Being

Included in the plan. Members get personalized daily tips, a health assessment, a preventive care checklist, integration with more than 100 tracking devices and apps, self-guided Journeys courses, healthy habit trackers, sleep and nutrition guides, and a tobacco coaching program.

Virtual Well-Being and the 24-Hour Nurse Line

Weekly live and on-demand webinars and guided meditations through Blue Cross Virtual Well-Being, registered nurses available around the clock to answer health questions, and the AHealthierMichigan.org blog on topics from mental health to workouts.

Blue365 Member Discounts

Blue Cross’s exclusive discount program, included with its medical plans, with savings on health-related products and services such as gym memberships, travel and personal care items. Categories include fitness, healthy eating, wellness and hearing and vision care.

Well-Being Plus and Custom Well-Being

Self-funded customers can enhance the included program with Well-Being Plus or Custom Well-Being reward programs.

Healthy Blue Living

A wellness incentive health plan in which employees improve their health to earn lower out-of-pocket costs. Blue Cross’s toolkit covers the health assessment, a Blue Care Network qualification form, weight management and tobacco coaching, with reminders at 90 and 120 days.

Blue Cross Complete of Michigan (Medicaid)

Blue Cross’s Medicaid plan, not an employer group plan, so an employer does not buy it. Its community resources page lists a ZIP-code resource hub for free or reduced-cost services, a Wellness and Opportunity Center in Detroit, free Blue365 discounts, Mission GED, a 24-hour nurse help line and virtual behavioral health urgent care.

Priority Health

PriorityWell pairs a wellness platform with a long list of optional programs.

PriorityWell is the digital health and wellness solution Priority Health launched in 2022 for large-group employers. Its provider manual, updated in July 2026, lists many more programs for commercial members, with eligibility that depends on the employer’s plan.

The PriorityWell Platform

Members get program recommendations based on their health needs, daily interactive content, healthy habit tracking and about 80 individual and group challenges on exercise, healthy eating and stress. Members earn incentives as they track activity.

PriorityWell Choice Benefits

Added in 2023. Members earn lower out-of-pocket expenses by completing activities that match their interests and goals, tracked on a Wellbeing Hub run by Virgin Pulse. Members can also earn Pulse Cash for gift cards, fitness accessories or charitable donations. Priority Health describes it as available to self-funded and fully insured employer groups.

Opt-In and Rider Programs

Priority Health’s own provider manual says that where commercial plans are marked eligible for a program, not every member plan qualifies, because employer groups opt in or out. Several programs below are optional riders or buy-ups, so confirm what a specific group has.

Diabetes and Weight Management

Virta offers commercial group members a diabetes reversal program and two weight loss programs, one of which adds GLP-1 prescribing, each with eligibility criteria. Livongo is available to groups that buy it, and Omada and the National Diabetes Prevention Program support members with pre-diabetes.

Behavioral Health and Mental Wellness

All members have free 24/7 access to licensed behavioral health clinicians, plus Teladoc Health Mental Health coaching tools. Commercial group members can use Boulder Care for substance use treatment, though some self-funded groups opt out. Backpack Healthcare covers pediatric and family mental health, and Folx Health offers virtual care for LGBTQIA+ members.

Muscle, Joint and Pain Care

Sword Health offers virtual physical therapy to groups that buy the rider. An optional Musculoskeletal Centers of Excellence rider covers joint replacement and spine procedures with reduced member cost share and travel coverage. Flowly is a virtual pain clinic, and the Garner Health supplemental product covers out-of-pocket costs for members who choose top-rated providers.

Maternity, Fertility and Family

PriorityMOM supports expectant mothers, and PriorityBABY supports families during a child’s first two years, with health information, gifts and incentives. Groups can add Carrot Fertility through a buy-up or rider for fertility treatment, pregnancy, postpartum, adoption and menopause support.

Fitness, Perks and Discounts

Commercial members can join Active&Fit Direct, with more than 11,000 fitness centers for $25 a month plus a $25 enrollment fee, according to Priority Health. Non-Medicare members also get BenefitHub perks, and non-Medicaid members get discounted hearing exams and hearing aids through TruHearing. Assist America provides global emergency medical services when traveling.

Cost Tools and Care Management

A Cost Estimator built with Healthcare Bluebook and a PriceMyMeds prescription price comparison for commercial members. Priority Health says all of its plans include care management at no cost for members with chronic conditions, and Ayble offers digestive health support to eligible group members.

Health Alliance Plan

A dedicated consultant, a customizable incentive and a full set of care programs.

HAP describes a well-being and health promotion team that supports employer groups through planning, implementation and evaluation. HAP also lists member programs whose eligibility varies by plan type and funding, so confirm what applies to a group.

Consulting and Programming

A dedicated well-being consultant, an employer interest survey, an annual strategy, incentive design support, on-site and virtual programming, behavior change programs and group-level reporting. HAP notes that some services carry a cost and some have minimum participation rates.

Reward Your Health

An incentive program aimed at health behavior changes and biometric improvements. The employer customizes eligibility, including whether covered spouses can take part, along with timelines, incentives and requirements.

iStrive and Quarterly Challenges

iStrive for Better Health is a digital wellness manager powered by WebMD Health Services, with a health assessment, daily habit tracking and rewards for activities and screenings. HAP says members have access at no additional cost. Quarterly challenges each focus on a new area of well-being, with prize drawings for those who complete them.

Wellness Wednesdays and Worksite Events

A live virtual webinar series on the third Wednesday of each month, covering stress management, healthy eating, financial wellness and more, with partners such as the American Institute for Preventive Medicine and GreenPath. HAP also offers on-site programs and worksite flu shot clinics.

Weight, Tobacco and Behavioral Health

Nutritional counseling, doctor-supervised weight loss programs and discounts. Tobacco cessation through the Michigan Tobacco Quitline, with four phone coaching sessions. A behavioral health team that matches members with mental health and addiction resources.

Maternity and Family Support

For commercial members, a maternity care management program with ProgenyHealth, including a nurse care manager, the Ovia Health app and free dental services during pregnancy. Pregnant members get an expanded nine-call tobacco program, and HAP runs a postpartum depression screening program.

Condition and Care Management

A care management program open to all members, transitional care after a hospital stay, a digital back and joint pain program with Protera Health for commercial and self-funded groups, and Hospital at Home for eligible fully insured and self-funded Henry Ford Health members. HAP also lists an antidepressant medication program for employer group members.

Preventive Reminders

HAP sends reminders during the year for vaccines, well-child visits, mammograms, cervical and colorectal cancer screening, diabetes care and physicals.

UnitedHealthcare

Rewards, fitness, Care Cash and clinical support for employer plans.

UnitedHealthcare lists a long set of programs for members on employer plans. Eligibility varies by plan type, funding arrangement and any buy-up, so confirm what applies before you promise a program to employees.

UnitedHealthcare Rewards

Included at no additional cost on many employer plans, with the amount and eligibility varying by plan type. Employees earn dollars for activities such as tracking steps, active minutes and sleep, or completing a biometric screening or health survey. UnitedHealthcare says employees can earn up to $1,000 a year, and that the annual amount varies.

One Pass Select

A subscription fitness program with a nationwide gym network, digital fitness options and grocery delivery where the state allows it. There are no long-term contracts and employees can change tiers monthly. Family members aged 18 and over can be added.

Real Appeal Weight Loss

An online weight loss program built around a health coach and a home Success Kit. UnitedHealthcare offers it to eligible members on employer plans.

Quit For Life and Wellos

Quit For Life gives tobacco users one-on-one coaching, digital tools and support for nicotine replacement therapy, depending on eligibility. The Wellos app adds health tracking, coaching and a library of more than 5,000 recipes for meal planning.

Care Cash

On eligible plans, members receive a prepaid card, listed at $200 for an individual or $500 for a family, when they use designated care. That includes UnitedHealth Premium Care providers, in-network primary care, outpatient behavioral health, labs, convenience clinics, 24/7 Virtual Visits and urgent care.

Condition Management Programs

Clinical support covers diabetes (app, biometric sensors and coaches), COPD, coronary artery disease, heart failure and asthma. Resource services are also listed for congenital heart disease, bariatric surgery, transplant and kidney care, plus Cancer Resource Services, which uses a 37-center centers-of-excellence network.

Maternity and Fertility

Digital maternity support, doula services and NICU Neonatal Resource Services. Fertility Solutions guides members through treatment, and Fertility Solutions Plus adds a virtual clinic through Maven. Availability depends on the plan.

Navigation and Inclusion Support

Naviguard helps members with out-of-network billing questions. UnitedHealthcare also lists LGBTQ+ resources. This page covers employer plans only. Medicare Advantage, Medicaid, ACA individual and Health Perks programs are excluded.

Aetna

Health Your Way, consulting, discounts and women’s health support.

Aetna pairs a configurable well-being platform with consulting and a broad set of condition programs. Which pieces apply depends on plan, funding and what the employer adds.

Aetna Health Your Way

The core well-being platform covers more than 40 topics with whole-health coaching. It personalizes what each member sees using claims, assessments, connected devices and biometrics, so outreach follows the person rather than a generic calendar.

Flexible Program Design

Employers can combine biometric screenings, incentives, group sessions and one-on-one coaching, delivered digitally or on site. Aetna says the framework fits different population sizes and budgets.

Well-Being Consultants

Aetna well-being consultants help build short-term and long-term strategies, train well-being champions at the employer and develop an annual well-being calendar. Aetna also runs a Workplace Well-Being Award.

Reported Results

Aetna reports outcomes from its 2025 book of business: 63% of members reduced blood pressure, 53% lowered cholesterol, 44% improved stress management and 52% reduced sick days, with more than 2.3 million coaching topics completed. These are Aetna figures, and results for a single employer will differ.

Member Discounts

Discounts on weight-loss and meal plans, gyms and fitness, massage, acupuncture, chiropractic and nutrition, vision (including EyeMed and LASIK) and hearing (Hearing Care Solutions and Amplifon). These are discounts, not insurance coverage.

Women’s Health and Fertility

Family planning and fertility support, including Infertility Institutes of Excellence and coverage for IUI where the plan includes it. Allara Health is listed for women’s health conditions, and preventive screenings are part of the plan design.

Maternity Support

Specially trained nurses, a lactation network and the option to add Maven for virtual maternity and family care. Availability depends on the plan and any add-on the employer selects.

Menopause Support

Aetna lists MinuteClinic, Gennev and Midi as access points, along with trained care managers. Employers can add coaching through ActiveHealth and Maven.

Chronic Condition and Cancer Support

Support for more than 35 chronic conditions, plus cancer support, diabetes management and joint support. CVS and MinuteClinic integration gives members in-person and virtual access points for routine care.

Cigna

Healthy Rewards, incentives, coaching and behavioral health.

Cigna describes its wellness offering as a mix of included programs and add-ons. Availability depends on group size, plan and employer selections, and Cigna directs employers to their representative or broker for specifics.

Healthy Rewards Discounts

A discount program covering fitness, healthy eating, personal care, wellness, hearing and vision. Members use it without claim forms or referrals. Like other carrier discount programs, it is not insurance coverage.

Health Matters

Starts with a health assessment and connects to incentives through the myCigna platform. Employers also receive enhanced client reporting.

Personify Health Well-Being Solution

Cigna lists a well-being solution with Personify Health, included for businesses with 5 to 499 employees.

MotivateMe Incentives

An incentive program that Cigna lists for employer plans. Reward design affects which wellness-program rules apply, so plan design and compliance review are part of the setup.

Health Advisor Coaching

Cigna lists Health Advisor coaching for members working on their health goals. Which coaching options apply depends on the plan.

Behavioral and Mental Health

Cigna offers behavioral and mental health programs alongside medical coverage. Which ones apply depends on the plan and employer selections.

Employee Assistance Program

Cigna states that 100% of EAP services are delivered by in-network behavioral providers. Cigna cites a 24% reduction in avoidable ER visits from its 2020 EAP study. This is Cigna’s own finding.

Pathwell Bone and Joint

Designated providers, clinical navigation and a benefit design that rewards members for choosing optimal care for bone and joint needs.

Integrated Medical, Pharmacy and Behavioral

Cigna cites a 2024 study showing $193 per member per year in medical savings when medical, pharmacy and behavioral coverage are all with Cigna. Cigna notes that results vary. Ask the Cigna representative or broker which programs are available to a given employer.

McLaren Health Plan

Self-management tools, health system events and care management.

McLaren’s published wellness resources are lighter on rewards and incentives than the larger carriers, and lean on its own health system. Confirm what applies to a fully insured or self-funded McLaren group before you rely on it.

Plans and Funding

McLaren Health Plan sells fully insured HMO, point-of-service and high-deductible plans to small and large groups, and its subsidiary McLaren Health Advantage administers self-funded plans for employers with 50 or more employees. Wellness features can differ between the fully insured and self-funded products.

Self-Management Tools

Interactive tools that help members check risk factors, get guidance on health issues and track toward lower risk. Members can also call the plan’s nurse support line for more help.

McLaren Health Care Events

Educational classes, support groups, screenings, seminars and special events run through the McLaren health system. McLaren Health Plan points members to its events calendar.

Condition and Complex Care Management

The plan publishes member information on adult and child complex care management, asthma and diabetes, including living with diabetes and diabetes with psychiatric medications. Health Advantage describes disease management programs as part of its PPO.

Preventive and Behavioral Health Resources

Members are encouraged to get preventive screenings when their doctor recommends them. The plan links to behavioral health self-screening tools, PrEP and PEP information, and guides on tobacco cessation, healthy weight, physical activity, healthy eating, stress and at-risk drinking.

McLarenNow Virtual Care and Pharmacy

McLarenNow Virtual Care visits are promoted to Health Advantage members. KSP Specialty Pharmacy, McLaren’s specialty pharmacy, offers mail order for specialty and brand name medications.

Humana

Go365 is Humana’s wellness program, but employer medical is gone.

Humana publishes a workplace wellness program called Go365. Whether a group can still buy it depends on its Humana coverage, since Humana has left employer group medical. Humana’s exit notice →

Go365 Workplace Wellness

Humana describes Go365 as an employee wellness program powered by Humana, with a customizable experience for companies of any size.

Incentives Built on Behavioral Science

Go365 rewards employees for healthy activities and gives them tools and support along the way. Humana says the program is rooted in behavioral economics and actuarial science, with feedback, goal setting and education components.

Whole-Person Resources

Beyond physical health, Go365 offers resources for everyday stressors such as financial, family and emotional concerns, which sets it apart from a steps-and-screenings program.

Customizable for the Employer

Employees use it on a smartphone, tablet or computer, and Humana says the experience can be customized to each company. Employer examples Humana publishes include a city government and a school system, which reported that it brought departments together through walking groups and fitness programs.

Go365 for Medicare and Medicaid Members

Members earn rewards for preventive screenings, verified workouts and community activities, and redeem them for gift cards in the Go365 Mall. Humana lists this version for eligible Medicare Advantage members and for Humana Healthy Horizons Medicaid members, and says it is not available with all Humana plans.

Confirm Availability Before Relying on It

Humana announced in 2023 that it was exiting employer group commercial medical plans, including the associated wellness and rewards programs, and its workplace Go365 page is not indexed for search. Humana still sells group dental, vision, life and disability. Confirm with Humana whether Go365 is open to a given group, and ask us about the few Michigan groups whose filings still list Humana medical.

What To Ask

Three questions to put to any carrier.

Who Funds the Incentive?

Find out whether rewards come from the carrier, from the plan design or from your own budget. The answer changes what the program costs you each year.

What Does Our Size Qualify For?

Some programs are described for large groups, for self-funded plans or for certain headcounts. Ask which tier your group can actually buy.

What Will We See Back?

Ask what the carrier reports to the employer and how often, and whether results can support a premium rebate under Michigan law.

Not Covered Here

A note on Total Health Care.

Total Health Care, the Detroit HMO, was acquired by Priority Health and is now a Priority Health subsidiary, so its members fall under the Priority Health programs above. Ask us and we will pull current details for your group.

Questions We Get

What employers ask about carrier wellness programs.

Are wellness programs included in a health plan?

Often a basic platform is. Blue Cross says its Well-Being program is built into every BCBSM and Blue Care Network medical plan, and Cigna says its well-being solution with Personify Health is included for businesses with 5 to 499 employees. Incentive and reward programs are more often an add-on or limited to certain group sizes or funding types.

Do carrier wellness programs lower an employer’s premium?

Not automatically. Most carrier programs deliver engagement tools and rewards to employees. Michigan law separately lets insurers offer premium rebates for an employer wellness program that shows results, within a 30% cap, or 50% for tobacco cessation.

Which carrier has the best wellness program?

We do not rank them. The right fit depends on your group size, whether the plan is fully insured or self-funded, and what your employees will actually use.

Do these programs create compliance obligations?

They can. Incentives tied to health outcomes fall under HIPAA and ACA wellness rules, and programs that collect health data can raise ADA and GINA questions. Have employment counsel review the design.

What do employers see about individual employees?

Carriers describe employer reporting in aggregate terms, such as group-level engagement data. Individual health information is protected, and an employer should confirm what a program reports before launch.

Can an employer use a wellness vendor instead of the carrier’s program?

Yes. Some employers run a stand-alone vendor program. The carrier’s program is usually easier to connect to the plan’s benefits, so compare the two before deciding.
